COLISTIM
COLISTIM (Colistimethate Sodium) [1MIU equivalent to 80 mg] Powder for solution for Injection, Infusion or Inhalation.

Colistim contains the active substance colistimethate sodium. Colistimethate sodium is a narrow spectrum antibiotic. It belongs to a group of antibiotics that are called polymyxins. Colistim is effective against most Gram-negative bacilli. Colistim works by killing some types of bacteria that can cause various sorts of infections in people. Like all antibiotics, Colistim is only able to kill some types of bacteria mainly Pseudomonas aeruginosa, Klebsiella pneumoniae and Acinetobacter Baumanii. It is considered to be the last resort antibiotic.
Indications:
Colistim can be used to treat infections in adults and children who have cystic fibrosis, post-surgical Meningitis, UTIs, SSTI, sepsis, IAIs, VAP where all antibiotics have failed. Colistim is not often used to treat these types of infections but may be used when other antibiotics are not suitable for some reason.
Pharmaceutical Form:
1 MIU/Vial: Sterile white powder in a 10ml colorless glass vial with a transparent 'flip-off' cap.
Reconstitution:
1 MIU Vial: 2 ml Sterile Water/Saline.
Dosage Guidelines:
For use by Infusion or Injection:Adults and adolescents:
- Maintenance dose 9 million IU/day in 2-3 divided doses.
- In patients who are critically ill, a loading dose of 9 MIU should be administered.
- Modelling suggests that loading and maintenance doses of up to 12 MIU may be required in patients with good renal function in some cases.
Children ≤ 40kg
- 75,000-150,000 IU/kg/day divided into 3 doses.
- For children with a body weight above 40 kg, use of the dosing recommendation for adults should be considered.
- The use of doses >150,000 IU/kg/day has been reported in children with cystic fibrosis.
- When Colistim is to be given by inhalation using a nebulizer, the usual dose for children under two years of age is 500,000 to one million units given twice a day.
- In most older children and adults, the dose is one or two million units twice a day, with a maximum of two million units three times a day.
